CUSTOMER DECLARATION OF USE FOR PRODUCTS CONTAINING NITROUS OXIDE

By this declaration, I promise that I am well aware that the product I am purchasing contains Nitrous Oxide(N2O) gas, and I will use the product in legitimate purposes, for catering purposes and in maternity wards when used as pain relief during labour.

I confirm that I am the named purchaser on this order, and that both the named purchaser & recipient are over the age of 18, and that I am able to provide proof of age if requested.

Statement 1

I understand that the item I purchased contains Nitrous Oxide and that inhalation of Nitrous Oxide carries a risk of serious health consequences. The risks of such misuse include narcosis, asphyxiation and potentially death.

Statement 2

I confirm that I am the named purchaser on this order, and that both the named purchaser & recipient are over the age of 18, and that I am able to provide proof of age if requested.

Statement 3

I confirm that I do not intend to misuse the Goods for their psychoactive effects, or supply the items to anyone who is believed (i) may be under 18 years of age, or (ii) may misuse the items for their psychoactive effects

Statement 4

I will take all reasonable measures to ensure that the products are used only in accordance with the instructions (lawful for catering purposes and in maternity wards when used as pain relief during labour) and will be kept secure so that they cannot be misused by children or others.

Statement 5

I will ensure that if reselling your products that any signage, labelling or information which relates to or accompanies the products explains it is for catering purposes and in maternity wards when used as pain relief during labour and does not suggest any other use. I confirm that I am aware of the HAZARDOUS nature of Nitrous Oxide and will not interfere with the packaging to obscure safety information and will take all necessary measures with regard to the storage and transportation of the cartridges. The cartridges will NEVER be put aboard an aircraft, heated in any way or pierced with anything other than a cream whipper.
